Community Care Planner

Somerset Care is hiring for a Community Care Planner to support our care staff working across the local communities. This essential role acts as a resource planner, coordinating where our care teams need to be each day, in order to provide the invaluable care and support that our customers require that enables them to live the life they choose.
Based in our Melksham office just off of Commerce Way, this Community Care Planner role will support our Community Carers who work across Wiltshire. This is a full time position working 39 hours per week with shifts based over a 2 week rota, working 6.30am to 3pm and 9.30am to 6.00pm with an alternate Saturday.
About the role:
You’ll be joining a fantastic Community team managed by the Community Services Manager, who work together to ensure every customer in our care gets the support they need in the least intrusive way, putting their comfort, happiness and safety first. We pride ourselves on delivering exceptional person centred care and making a real difference to the lives of those we support. This role plays an intrinsic part in ensuring this can happen.
As a Care Planner you will provide high level resource planning with duties including, managing and planning staff rotas, ensuring resource is appropriately deployed and adequate staffing levels are maintained. You’ll maximise the quality time our carers have with customers by minimising their travel time and meticulous planning, ensuring the continuity of care wherever possible. You’ll manage daily changes including annual leave and absence whilst always ensuring staff skills are matched against the needs of each customer.
You will enjoy resolving queries quickly and effectively and be confident using your own initiative. Experience of dealing with sensitive data and an understanding of Information Governance/Data Protection is essential, whilst experience in a care setting or resource planning would be desirable
You’ll have an excellent telephone manner, with strong communication skills and a ‘can do’ attitude with a friendly manner and natural desire to help. You’ll be a real ‘people person’, able to build lasting relationships and love working in a team
A full induction course and ‘shadow’ training will be provided to successful candidates.
