Intensive Community Navigator

Open Mental Health is a Somerset alliance of local voluntary organisations and the NHS. We are working in partnership to ensure that residents of Somerset get the support they need, when they need it. Our shared ambition is to ensure that people living with mental health problems get the right support at the right time.

Working together, we support people to live a full life, by enabling access to specialist mental health services, housing support, debt and employment advice, volunteering opportunities, community activities and physical exercise, to help support and improve their wellbeing and quality of life.

 

Intensive Community Navigators x 2 roles

Hours per week: 37

Contract: Fixed-term 6 months

Salary: £21,718 – £23,053

Location: Somerset (traveling will be essential)  

Hours: Monday-Friday with the expectation that evening & weekend work will be required.

We are establishing a new team in Somerset in partnership with Mind and other VCSE organisations. It is an Intensive Transition Team to support timely and safe hospital discharges for people with mental health needs. The key elements of the role are:

  • To support safe discharges by building strong and trusting relationships with clients.
  • To in-reach into hospitals and worth with statutory colleagues to complete & implement recovery plans
  • To navigate services around the client to ensuring agencies are coordinated around the individual. You will do this through engaging in multi-agency discharge meetings.
  • To deliver trauma and psychology informed support and using person-centred support plans and tools including: wellness recovery action plans.
  • To maximise opportunities and access to accommodation, community networks, volunteering & employment.

We are looking for people who have experience of working with people with long term mental health and complex needs and who have excellent communication skills.
