Locality Lead – Community Team

We are continually developing new ways of working using a strength-based approach and involving all our System partners working towards a Neighbourhood approach to support and strengthen our Communities in Somerset.

Building on Somerset’s Intermediate Care Model our Social Care Practice continues to embrace a community focus with our Community Locality Teams supporting an individual discharge from hospital, providing continuity of support and intervention. Our Commitment remains in promoting independence enabling individuals to remain living in their own homes for longer.

Here’s what you can expect to be doing

As a Leader in the Locality, you will work closely with your Locality Manager and your social care colleagues to lead a multi-disciplinary team, motivating and managing all members to ensure that their work is of a consistently high standard.

Promoting independence should be the driving factor in everything your team does, and we’ll look to you to ensure that this is always the case.

You will have the opportunity to working alongside Health and Social Care colleagues to shape how services in your area are delivered, designing the way that your team operates and bringing in creative ways of working.

A few things about you

You’ll be driven by your passion for making a genuinely positive difference to the lives of individuals, families and carers. This will be backed up by your considerable knowledge and experience of Community Social Care practice – allowing you to provide clear direction, support and mentoring to your team.

An integral part of this role is providing clear and inspirational leadership for your team – it is therefore essential that you have some experience of leadership or aspire to developing this. You should have a good understanding of data and be able to use this to evidence practice, performance and development opportunities.

You’ll be brave in your approach to work, not afraid to do things differently where it genuinely benefits the individual, carers and community. You’ll be able to demonstrate a real creative spark when it comes to your work.

Additional information

Full COVID-19 vaccination status will be required as a condition of employment. Recent legislation now mandates that anyone working in a CQC registered care home and those visiting in a professional capacity, must be fully vaccinated (MHRA approved) by 11 November 2021 unless a medical exemption applies.
