We’re looking for people to support us to deliver a new service in primary care as part of the Pier Health social prescribing service. We are looking for a Senior Recovery Navigator, who will ensure people with moderate-serious mental health needs are supported to connect to their community, to build resilience and to improve their wellbeing.
Working collaboratively with the client, GP, social prescribing team and other health & care professionals, they will provide practical and emotional support that is tailored to the individual. This may include things like attending an important appointment with them, helping them fill out a form or giving them information about other services.
You’ll have experience of working with people with long term mental health and complex needs and carrying out needs and risks assessments. You will need to take a holistic view, with a recovery and psychologically informed approach. You’ll be a good communicator, with experience of supervising an effective team and liaising with partners, to contribute to the delivery of this valuable service.
We welcome applications from Experts by Experience as well as underrepresented groups.
Second Step offer a blended approach to working – a combination of home, office and community-based working dependent on the needs of the clients you support; an excellent annual leave package; a comprehensive induction programme and on-going training opportunities.
