Social Worker – Preparing for Adulthood (Transitions team)
A bit about us
Somerset County Council have an exciting part time opportunity for an experienced Social Worker to join our Transitions team on a 30 hour per week basis. The Key purpose of the role is to improve the outcomes for our most vulnerable young people and families in Somerset to encourage positive change.
Here’s what you can expect to be doing
You will provide advice, guidance and support to young people and their families. You will be having conversations with them identify what outcomes they would like to achieve and what support they may require achieving them using a strengths-based approach. You’ll be able to advise on appropriate local services and solutions available to suit the individual’s needs. A key element to this position is to encourage our young people to be independent and to support them to grow in confidence whilst utilising the support available within their local community.
You will also work with a wide network of professionals from other agencies and organisations and work collaboratively to ensure the best outcomes for the young people you support.

With permanent Team Managers and a permanent stable senior leadership team you’ll be well supported with regular and reflective supervision.

A few things about you
You will be passionate about relationship-based social work, proud of your profession, and able to reflect and think creatively about how you can build relationships to make a difference to young people and families.
You will need to be a qualified social worker registered with Social Work England and have substantial experience of the Care Act 2014, Mental Capacity Act and of working with people with learning disabilities. Having knowledge and/or experience of the SEND Code of Practice and Education Health and Care Plans would also be an advantage.

This role has a current salary of £30,451 to £34,728 (£24,690 – £28,157 pro rata).
